Schedule an Evaluation

To schedule an evaluation, please call our office at 907-452-4517.

Before Your Child’s Evaluation

  • • Referral Requirement: A current prescription/referral from your child’s pediatrician is required, specifying Speech or Occupational Therapy Evaluation and Treatment.
    • Tricare Clients: A current authorization must be in place before scheduling.
    • Fill out the registration paperwork:

    Complete the packet at home and email it to us via our encrypted email: talkabout@medsecuremail.com

    You may also print it out (single sided) and bring it to your appointment or pick up a physical packet in our office several days ahead of time.

    Insurance Information

    • • If you have commercial insurance (not Tricare or Medicaid), Please contact your insurance provider and gather the necessary details using this form:

    • • Out-of-Network Insurance: If Talkabout Inc. is not enrolled with your insurance company, we are required to provide a Good Faith Estimate (GFE) of potential charges:
